Safety Recognition  Award

ARM invites your participation in its Safety Recognition Award. The purpose of this award is to highlight the importance of safety in all aspects of our industry and to award the rotational molder member that has the lowest incident rate.

Many of you have already filled out a Bureau of Labor Statistics report called “OSHA Form 300A, Summary of Work-Related Injuries and Illnesses.” Submit a copy of your OSHA Form 300A, using 2009 data only (the award is based on cumulative data from all your plant locations). All information provided will be kept confidential and will not be published in any list by ARM. The intent is to request only data that is already provided to others, such as OSHA, so as not to create any additional work. (This award is open to all rotomolder members -- regardless of location.)

Entry Deadline: September 1 2010

  • OSHA Form 300 (Return only Form 300A, on page 8 of this PDF)
